Bedside charting from WhatsApp lets a nurse or doctor write an entry onto an admitted patient's chart — vitals, doctor's notes, nursing notes, IV fluid, feeding, nebulisation, oxygen, ICU or NICU monitoring, a procedure, the case history — by sending one typed or spoken message to the hospital's WhatsApp number. The AI extracts the chart's fields from the message, the same validator that guards the web form checks them, and the entry is saved on the patient's file with the time the nurse said, not the time the phone was free.
This guide explains the IPD Patient File that sits behind it, how the ten charts work from WhatsApp, the permission and versioning rules that make it safe, and what it changes on a ward round.
The IPD Patient File
Every Indian hospital's inpatient file is a stack of paper sheets — the case history, daily doctor's notes, nursing notes, the IV fluid chart, the feeding chart, nebulisation and oxygen records, ICU and NICU monitoring sheets, the procedure record. Healthixio's Patient File, modelled on a paediatric hospital's twelve-page paper file, puts all of them behind one tabbed screen per admission, with each chart defined as data: its fields, limits, who may write on it and how it prints. A summary sheet is derived from the record — doctor visits, ward transfers, receipts, charts and billed services — rather than typed a second time, and the case history pre-fills the discharge summary.
Charting from WhatsApp
Record Vitals
The nurse taps Record Vitals, picks the patient, and sends one line: "BP 130/80 pulse 88 temp 99 spo2 97". Hindi labels — नाड़ी, बुखार, ऑक्सीजन, वजन — are understood. The agent shows what it parsed, asks for a confirm, and saves through the same function the IPD vitals screen uses. It then goes back to the patient picker, because a round means twenty patients in a row.
Patient File
The nurse taps Patient File, picks the patient, and is offered only the charts they are permitted to write on. They send one message — "started NS 500 ml at 80 ml per hour at 2 pm, site right forearm, no swelling" — as text or a voice note. The AI extracts that chart's fields; a spoken time is honoured; choice fields are validated against the chart's options; anything not said is left blank. The entry is saved and appears on the web Patient File instantly.
My Round List
A doctor taps My Round List and receives every patient admitted under them with the latest vitals and when they were taken — the ward round on one screen before walking onto the ward.
The ten charts
| Chart | Typical entry from WhatsApp |
|---|---|
| Case History | Complaints, history, examination on admission — pre-fills the discharge summary |
| Doctor's Notes | Round findings, plan, orders |
| Nursing Notes | Observations, care given, patient condition per shift |
| IV Fluid | Fluid, volume, rate, start time, site |
| Feeding | Feed type, volume, route, time — paediatric and NICU |
| Nebulisation | Drug, dose, time, response |
| Oxygen | Mode, flow, SpO2, start and stop |
| ICU / NICU Monitoring | Hourly readings and supports |
| NICU Continuation | Ongoing neonatal observations |
| Procedures | Procedure, time, operator, notes |
A hospital that does not run paediatrics can hide the feeding and NICU charts; a paediatric hospital switches them on.
Permissions, versions and audit
- Role-based writing. Doctors write doctor's notes; nursing staff write nursing charts; reception covering a ward can be granted nursing-chart access explicitly. The chart list offered on WhatsApp is the list the person may write on, re-checked at the moment of saving.
- Nothing is edited in place or deleted. An edit creates a new version and retires the old one; a void keeps the entry with a reason. The chart always shows what was known when.
- Attribution. Every entry carries the author and the time, whether it came from the web form or from WhatsApp.
- Same validation everywhere. A field the web form would reject is rejected from WhatsApp too.
Those four rules are what NABH assessors, TPA auditors and, from May 2027, the DPDP Act's security-safeguard requirements ask for. See NABH and hospital software.
What changes on the ward
- Vitals are recorded when they are taken, not transcribed from a notebook at the end of the shift.
- Night entries are made by the person on the ward, from their own phone, without walking to the nursing-station PC.
- Doctors see the latest readings before the round and dictate their notes during it.
- The case history is already on the file when the discharge summary is dictated, so the doctor adds only the course and the advice. See voice-note discharge summaries.
- The printed case sheet at discharge is generated from the record, complete and legible.
Safety
The language model is used for one job — extracting a chart entry's fields from one message — and is told never to invent a value. A deterministic workflow decides what happens next; the chart definition decides what is valid; the person's role decides what they may write. A nurse cannot write a doctor's note, a message cannot reach another hospital's patient, and an entry can be voided but never made to disappear. Read what makes an AI agent safe.
Frequently Asked Questions
Can nurses record vitals on WhatsApp?
Yes. A nurse authorised in the HMS messages the hospital number, picks the patient, and sends vitals as one line of text or a voice note, in English or with Hindi labels. The entry is confirmed and saved to the patient's IPD chart.
Is WhatsApp bedside charting valid for NABH?
Entries carry author, time, version history and void reasons, are validated against the chart's rules, and print as part of the case sheet — the properties NABH looks for in a medical record. The medium is the chat; the record is the HMS.
What is the IPD Patient File?
Healthixio's digital version of the paper inpatient file: ten bedside charts behind one screen per admission, a derived summary sheet, role-based permissions and a printable case sheet. WhatsApp is one of two ways to write to it; the web form is the other.
Does it work for NICU and paediatric wards?
Yes. Feeding, NICU monitoring and NICU continuation charts are built in and can be switched on for paediatric hospitals.
