In 2026 Meta charges Indian businesses per WhatsApp message by category: roughly ₹0.86 for a marketing template, ₹0.115 for a utility template, ₹0.115 for an authentication template, and nothing for a service reply sent inside the 24-hour window a customer opened by messaging first — plus 18% GST on all of it, and whatever markup a provider adds. From 1 October 2026 service messages become chargeable beyond a monthly free allowance of 1,000 per business number, and utility templates sent inside the window are charged too. A hospital that designs for patient-initiated conversations still pays a few hundred rupees a month; a hospital that blasts templates pays thousands.
This guide gives the rate card, explains the categories and the window, works through what a 50-bed hospital actually pays with Healthixio, and sets out the design rules that keep the number small.
The India rate card
| Category | What it is | Meta rate per message (India, 2026) |
|---|---|---|
| Marketing | Promotions, offers, announcements — any template Meta classifies as marketing | ₹0.8631 |
| Utility | Transactional notices about an existing relationship: reminders, confirmations, alerts | ₹0.1150 |
| Authentication | One-time passwords | ₹0.1157 |
| Service | Free-form replies, buttons and lists inside the 24-hour window the customer opened | ₹0 today; from 1 Oct 2026, ₹0 for the first 1,000 a month per number, then the utility rate |
Rates are Meta's, exclusive of GST at 18%. Providers that resell the API typically add 10–30% on top, and some charge platform fees per month regardless of volume. Rates are revised by Meta from time to time — the marketing rate rose about 10% in 2026 — so a system should record the rate applied on each message rather than assume one.
The 24-hour window, in one paragraph
When a person messages a business number, Meta opens a 24-hour customer-service window. Inside it the business can send free-form text, reply buttons and list pickers as service messages. Outside it the business can only send an approved template — marketing, utility or authentication — and pays per message. Tapping a button on a template counts as the person messaging, so it reopens the window. That single rule is the whole economics of WhatsApp for a hospital: the more the patient and staff initiate, the less it costs.
What changes on 1 October 2026
- Service messages get a monthly allowance of 1,000 free messages per business phone number; beyond that they are charged at the utility rate for the country.
- Utility templates sent inside the window, previously free, are charged at the utility rate.
- The 72-hour free entry point for conversations started from a Click-to-WhatsApp ad or a Facebook or Instagram button is unchanged.
For a hospital the effect is modest. A busy OPD bot answering 3,000 patient queries a month would pay for 2,000 of them at ₹0.115 — ₹230. Hospitals sending utility templates inside an open window as a habit will notice more; the fix is to reply free-form when the window is open and reserve templates for when it is shut, which is how Healthixio already behaves.
What a 50-bed hospital actually pays
| Message | Volume per month | Category | Cost before GST |
|---|---|---|---|
| Evening-before follow-up reminders | 3,000 | Utility | ₹345 |
| Vaccination reminders | 400 | Utility | ₹46 |
| Leave alerts and decisions outside the window | 60 | Utility | ₹7 |
| OTPs (portal, ABHA, password resets) | 300 | Authentication | ₹35 |
| Patient self-service replies (queue, token, booking) | 4,000 | Service | ₹0 now; about ₹345 from Oct 2026 |
| Staff agent replies (booking, vitals, discharge, attendance) | 6,000 | Service | ₹0 now; about ₹575 from Oct 2026 |
| Total | About ₹430 now; about ₹1,350 from October |
Compare a single promotional blast of 5,000 marketing templates: ₹4,315 before GST, for one send. The lesson is not that marketing is wrong; it is that marketing is a different budget line from operations, and should be a deliberate decision each time.
Seven rules that keep the bill small
- Inbound-first. Make the patient or staff member send the first message — a QR, a poster, a button on an existing template — and reply inside the window.
- Buttons on every template. A tap reopens the free window, so everything after the tap is free. Healthixio's reminders carry Confirm, Reschedule, Cancel; the vaccination reminder carries Book Appointment.
- Utility, not marketing. Register reminders and alerts as utility templates. Meta reclassifies templates that read like promotions, so keep the wording transactional.
- Templates only when the window is shut. Try the free interactive message first; fall back to the template when Meta says the window has closed.
- Do not resend failures that will fail again. A number that is not on WhatsApp, or a person Meta has capped for marketing, costs money and quality rating on every retry. Retry only transient errors.
- Charge on delivered, not sent. Meta bills for delivered messages; on cold lists roughly one in ten is accepted and never delivered. A cost ledger that records on delivery matches the invoice.
- Record the rate on every message. Rates change; a ledger that copies the rate in force onto each charge reconciles to the paisa when the bill arrives.
How Healthixio tracks the cost
Every message Meta reports as billable is written to a cost ledger with its category and the rate card in force that day, so the hospital's spend is derived from delivery receipts, not estimated. Rate cards are versioned — an edit retires the old card rather than overwriting it — and a billable message with no configured rate is flagged rather than counted as free. Only five utility templates ever open a conversation on a hospital's behalf: the appointment reminder, the vaccination reminder, the leave alert, the leave decision and an optional shift reminder that is off by default because it is the one message the hospital initiates every working day.
Frequently Asked Questions
How much does WhatsApp Business API cost per message in India in 2026?
Meta's rates for India are about ₹0.8631 per marketing message, ₹0.1150 per utility message and ₹0.1157 per authentication message, before 18% GST and any provider markup. Service replies inside the 24-hour window are free until 1 October 2026, and free up to 1,000 a month per number after that.
What is changing in WhatsApp pricing on 1 October 2026?
Service messages get a monthly free allowance of 1,000 per business phone number and are charged at the utility rate beyond it, and utility templates sent inside an open 24-hour window become chargeable. The 72-hour free entry point from ads is unchanged.
Is WhatsApp cheaper than SMS for hospital reminders?
Per message they are similar — a utility template at about ₹0.115 against transactional SMS at ₹0.12 to ₹0.20 — but WhatsApp carries buttons, PDFs and Hindi text, is read far more reliably, and everything the patient does after tapping a button is free. Per outcome it is much cheaper.
Does Healthixio charge extra for WhatsApp?
The WhatsApp AI agent, reminders, report delivery and patient self-service are included in every plan. Meta's per-message charges are tracked message by message in a cost ledger so the hospital can see exactly what it spends.
